All forms are completed online through the Patient Fusion portal. Please follow steps below:

You will receive an email from Patient Fusion with the subject "Your Name, your health records are now available"

2. Click the Access your records link

3. When asked to Verify Identity

i. Enter the 8 letter Pin from the Appointment Reminder email or

ii. Request a security code via phone

4. Verify identity using Pin.

i. Enter Date of Birth and 8 Letter Pin

5. Set up Account

i. Create new account or link to an existing Patient Fusion Account

6. Set Security Question and Create a password - Your username is your EMAIL Address

8. Select the Appointments tab on the left of the loading page

9. Select the "Complete check-in" tab to begin
